EXCEPTIONAL SERVICE

We ensure that we meet Service User needs by providing suitable person centred care whenever needed. The care provided will be reviewed periodically to ensure it remains fit for purpose.

EXCEPTIONAL SERVICE

All individual rights will be respected, with their individual living skills supported developed. Partnership working will be done with the Service Users together with their family members or any other nominated individuals to enhance Service User quality of life.

CUSTOMISED CARE PLANS

Personalized care planning is a patient-centered, whole-person approach to treatment planning. Personalized care plans improve patient outcomes and are now mandated for chronic care management reimbursement.
